Job Overview:
San Diego Detox is a premier substance abuse detox and residential treatment facility dedicated to providing compassionate, evidence-based care for individuals seeking recovery from addiction. Our multidisciplinary team is committed to supporting our clients through every step of their journey towards lasting sobriety and improved mental health. We are seeking a dedicated Licensed Vocational Nurse to work the NOC shift (7pm-7:30am). The ideal candidate will provide direct patient care under the supervision of registered nurses or physicians, ensuring the well-being of patients in various healthcare settings.
Duties:
● Execute the Medical portion of the client intake/admission procedure; e.g., the DHCS medical questionnaire, pain screen, & inventory on intake medications.
● Carry out routine program medication management tasks in a safe and effective way whilst completing records & documentation in the EMR system.
● Report any medication incidents (i.e., adverse reactions or observed side effects,
medication errors, non-compliance, or audit discrepancies) to the Head Nurse.
● Document medication related events and incidents on a Medication Event Report form.
● Review client responses in the DHCS Health questionnaire and address any medical issues
or concerns by asking follow-up questions if necessary.
● Routinely observe clients in the Detoxification program and take note of any medically
relevant observations; report any concerns to the Head Nurse.
● Ensure clients in the Detoxification program are stable and comfortable; document any
observations such as withdrawal symptoms or noteworthy medication side effects.
● Safely facilitate medication observation in accordance with the San Diego Detox LLC Medication Management program, adhering to DHCS and TJC standards.
● Perform COWS and CIWA assessments on clients in detoxification per company procedure and at designated intervals.
● Audit Medications regularly and document audit results, reporting discrepancies.
● Document client reactions to medications, including any side effects, intended effects, adverse reactions, and any other observed responses to medication.
● Document medication non-compliance and errors on Medication Incident/Event Report.
● Work with the pharmacy to assure that all client medications are processed with insurance
when applicable and that credit card authorizations are on file.
● Monitor and document client vital signs at designated intervals and as needed.
● Complete healthcare paperwork, such documentation of client interaction, vitals, etc.
● Maintaining client records / focus on documentation within the medical tab.
● Perform Waived Testing Urinalysis; collect urine samples, review POC results, enter in EMR
system in client chart prepare package for shipping to laboratory for analysis.
● Review client DHCS state questionnaires and address any medical issues.
● Report directly to the Head Nurse; perform other duties as assigned by the Head Nurse.
Qualifications:
●Hold a valid, active, unexpired and unrestricted Vocational Nursing License (LVN) credentialed through the Accreditation Commission for Education in Nursing (ACEN), or The Commission on Collegiate Nursing Education (CCNE)
● Current CPR/First Aid, renewed every two years
● Current TB Clearance (Negative PPD Skin test), renewed annually
