Job Title: Licensed Practical Nurse (LPN) - Skilled Rehab Nursing Home
Job Summary: The LPN will be responsible for providing nursing care and services to patients in a skilled rehab nursing home setting. They will work under the direction of a registered nurse or physician and will be responsible for administering medications, monitoring patients' vital signs, providing wound care, and ensuring that patient care plans are followed.
Key Responsibilities:
- Administer medications and treatments as ordered by physicians
- Monitor patients' vital signs and report any changes to the registered nurse or physician
- Provide wound care and assist with dressing changes
- Assist with diagnostic and therapeutic procedures
- Document patient care accurately and timely
- Follow established infection control and safety policies and procedures
- Provide education to patients and families about medications, treatments, and care plans
- Work collaboratively with other healthcare team members to provide high-quality patient care
Qualifications:
- Current LPN license in the state of employment
- Minimum of 1 year of experience in a skilled rehab nursing home setting
- Ability to work effectively in a fast-paced environment
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic computer skills
- Ability to lift up to 50 pounds and perform physical tasks such as standing, walking, and bending
Working Conditions: The LPN will work in a skilled rehab nursing home setting, They may be required to work weekends, holidays, and different shifts as needed.
