Qualifications: (Minimum)
- High School Diploma and 3 years’ experience in social/human services field and or pursuing a two-year associate degree in social work, human services, or another related field
- Expertise in case management and client advocacy
- Ability to work with youth with behavioral/mental health diagnosis and in critical and emergent crises situations Sensitivity to cultural and economic differences
- Knowledge of regional and service pool
- Thorough understanding of medical and psychosocial issues related to youth in residential care (RC) and Addiction
- Valid Pennsylvania Driver’s License
Responsibilities:
- Perform youth/family needs assessment, based on intake materials and youth interview
- Ensure billing is accurate for each youth on your caseload
- Provide case management/care coordination and advocacy by carrying a caseload and assisting youth/family in negotiating social services, providing follow-up and monitoring support. Reassess youth needs as appropriate
- Maintain contact with all outside providers involved with the youth/family. These include Court, CUA, DHS, Parent, School, medical etc
- Meet with youth weekly for individual case management concerns or follow-up. This includes status of school and discharge plan
- Assist residents with job applications and applications to vocational and educational training programs as needed
- Meet weekly with all youth on caseload to address case management vocational and/or employment goals
- Attend court hearings for clients related to criminal justice and family related issues
- Consult with the treatment team in formulating discharge planning
- Finalize plans for referrals for therapeutic support
- Complete continuing care plan with each resident, 2 weeks prior to discharge from treatment
- Lead weekly check out meetings
- Attend weekly individual supervisory meeting with the supervisor
- Maintain accurate, timely records of time spent with client, problem areas, problem resolution, etc
- Participate in staff development as required
- Provide and/or coordinate youth transport to social services and medical appointments according to program phases
- Coordinate and monitor client follow-up on progress after treatment: 7th days, 14th days, 30th days 90th days and 6 months
- Co-facilitate specialized client groups including conflict resolution, life skills and other duties as assigned
- Work with outside support team to develop discharge housing options for youth monthly and follow-up throughout the youth’s’ treatment stay
- Complete case management chart audit on a quarterly basis
- Other duties as assigned
