Duties include, but are not limited to:
- Making observations and thoroughly documents care and the patient's response
- Completing all clinical reminders that may be completed by the LVN prior to the due date and closes encounters
- Appropriately seeking assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention
- Performing, reporting and recording assigned nursing care in Primary Care Clinics, including, but not limited to:
- Assisting in determining the physical and mental health status, needs, and preferences of culturally, ethnically, and socially diverse patients and their families based on interpretation of health-related data. Performs focus assessments.
- Data collection and information, recognizing changes in conditions and documenting all interventions completed during patient visits. Reporting to the RN or another appropriate clinical supervisor to assist, review, and develop goals and plan of action.
- Administering and documenting immunizations per protocol/VATVCBHCS policies
- Clinical reminder completion
- Documenting transfers in CPRS and warm handoff reports given to receiving facility
- Closing encounters
- Completing pre-appointment telephone calls for scheduled patients
- Reviewing and assisting Team to address/complete pending alerts communicated on message manager throughout the day to assist the team to facilitate care and address needs of the caller
- As assigned, assuring twice daily documentation of temperature for medication refrigerator
- Answering telephone in a courteous and assistive manner, obtaining necessary information, communicating messages and/or referring to the appropriate team member for disposition
- Measuring and recording vital signs and enters them in electronic system
- Observing physical and/or behavioral changes in the patient condition and communicating these observations in the appropriate format.
- Providing and documenting instruction to patients on specimen collection and test preparation and initiating basic health teaching and/or reinforcing patient education under the guidance of the RN.
- Demonstrating flexibility and adapting to work demands or adjustment of assignment
- Performing other tasks as requested by providers or supervising nursing personnel within the scope of training and abilities
Work Schedule: Fulltime, Monday thru Friday 8:00 a.m. to 4:30 p.m.

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior work experience or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66

Physical Requirements: moderating lifting (15-44 pounds), moderate carrying (15-44 pounds), straight pulling (1/2 hour), pushing (1/2 hour), reaching above shoulder, walking (5.3 hours), standing (5.3 hours), repeated bending (5.3 hours), ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, depth perception, ability to distinguish basic colors and hearing (aid may be permitted).
