Position Summary:
The Care Coordinator ensures that participants are connected to resources in the community as determined by the Nurse Practitioner or other social determinants of health needs. This position also has the unique opportunity to support efforts to find new and innovative ways to provide safe and equitable healthcare services. The Care Coordinator will participate in a collaborative practice environment and contribute to program development and quality improvement.
Duties and Responsibilities:
Administration
o Manages assigned caseload and documents case notes in electronic system.
o Follows up on provider referrals for care coordination.
o Schedules and confirms client appointments.
o Identifies new resources and community programs available to participants.
o Collects data on social determinants of health that impact clinic participants.
o Collaborates with advocates, case managers, and coordinators in other Our House departments.
o Follows up on participant eligibility for services.
o Aides participants in transitioning to a long-term medical home.
o Performs opening and closing procedures for clinic, as needed.
o Trains and oversees volunteers or interns, as needed.
o Assists the Director of Health Programs with program projects.
o Addresses patient and provider inquiries via phone, text, and/or email.
o Processes laboratory reports, as needed.
o Inventories and orders necessary supplies.
o Participates in case conferencing.
o Manages front desk check-in and registration procedures, as needed
Clinical
o Delivers direct clinical services as needed, including obtaining vital signs, phlebotomy, collecting laboratory specimens and individual health education.
o Participates in continuing education activities.
o Enters documentation in electronic health record.
o Processes lab specimens and arranges for specimen pick-up
o Collects specimens for analysis.
o Performs point-of-care testing procedures.

Qualifications:
Education:
Minimum of an Associate’s degree in a health related field and American Heart Association BLS certification for healthcare professionals required. MA, CNA, or PCT certification preferred.
Experience:
One year of experience in a healthcare care setting as a care coordinator or case manager required. Prior experience working with vulnerable populations in a healthcare setting or in a free/charitable clinic and/or front desk/receptionist experience preferred. Community Health Worker experience preferred.
Bilingual in English and Spanish preferred.
